Combining the commissioning capability of primary care with the Quality & Outcome Framework provides a powerful tool for improving the services being delivered for managing long-term conditions such as diabetes & CHD. That is why, in this year's 2008 Acorn, QOF & Guy Rotherham Awards, the NHS Alliance, the Improvement Foundation, British Cardiac Society, Primary Care Diabetes Society (PCDS) & Primary Care Cardiovascular Society (pccs) have recognised the importance of commissioning by including a category for GP Commissioners & a new QOF Awards for Diabetes. 

The Secretary of State for Health has been invited to present all the awards during the NHS Alliance Conference on 16th & 17th of October 2008 in Bournemouth. This year there are a total of 14 categories of Award, 9 Acorn Awards, 4 QOF Awards and 1 Guy Rotherham Award, please see individual award categories below for details. 

QOF Awards - CHD & DIABETES
The Quality and Outcome Framework (QOF) for the GMS and PMS Contracts is a powerful tool for improving the services being delivered by general practice. The quality indicators are evidence-based and will improve Chronic Disease Management in primary care: a key priority for the NHS and PCTs in their Public Service Agreements. The organisers and judges are very pleased that Schering Plough are again sponsoring and supporting the QOF Award for CHD, and new for 2008 an award for Diabetes and a new category for GP Commissioning Consortiums. 

Entry criteria:CHD GP category
The CHD QOF Award is open to all GP practices in England, Scotland, Wales and Northern Ireland. A GP practice can enter themselves or alternatively a PCO can enter on behalf of a practice (after receiving the consent of the practice). All GP practices that have an actual percentage achievement exceeding the target percentage covering CHD, cholesterol and Heart Failure targets are eligible to enter. To be considered for this award, please complete the entry form and submit one page of A4 on how these targets were achieved, highlighting any demonstrable benefits for patient care & services. 

CHD Commissioning Consortium Cluster (GPCC) Category
The GPCC category is only applicable in England. A GPCC can enter themselves or, alternatively, a PCO can enter on behalf of a GPCC (after receiving consent of the GPCC). This category is open to GPCCs in England that have achieved an above average score across all its practices for the actual percentage achieved covering the CHD, cholesterol & heart failure domains. To be considered for this award, please complete one A4 page on how practice based commissioning, either by the GPCC being a commissioner, provider or both, improved the services in primary care for the management of CHD & heart failure. 

DIABETES GP Category
The Diabetes QOF Award is open to all GP practices in England, Scotland, Wales and Northern Ireland. A GP practice can enter themselves or alternatively, a PCO can enter on behalf of a practice (after receiving the consent of the practice). All GP practices that have an actual percentage covering diabetes targets are eligible to enter. To be considered for this award, please complete the entry form and submit one page of A4 on how these targets were achieved, highlighting any demonstrable benefits for patient care & services.  

DIABETES Commissioning Consortium Cluster (GPCC) Category
The GPCC category is only applicable in England. A GPCC can enter themselves or, alternatively, a PCO can enter on behalf of a GPCC (after receiving the consent of the GPCC). This category is open to GPCCs in England that have achieved an above average score across all its practices for the actual percentage achieved covering the diabetes domains. To be considered for this award, pleas completes one A4 page on how these above scores were achieved and how practice based commissioning, either by the GPCC being a commissioner, provider or both, improved the services in primary cars for the management of diabetes.

Winners Award
The winning entries will receive a free entry (for three members of the practice or PCO) to the NHS Alliance Conference and presentation of a QOF Award certificate by the Secretary of State for Health with Award Winners photographs. Winners will also be able to attend the NHS Alliance Conference dinner on 16th October along with the other winners, sponsors, organisers and the panel of judges. The opportunity for the winning practice or PCO to feature as a case study in the Award Winners Publication will also be provided. Finally, winners will receive an invitation to attend and perhaps speak at the follow-up VIP, MP & Press event at the Palace of Westminster on the 3rd December 2008 at which the Minister of State for Health has been invited to attend and speak.
